National American Heart Health Awareness Month

In Lieu of February being National American Heart Health awareness month we would like for you know we offer a Cardiac Care Program for those seeking rehabilitation with cardiac related conditions from home or following a hospital stay. With a patient centered approach to Cardiac Care we focus on:

• Improving the patient’s over all health
• Preventing avoidable re-hospitalizations
• Getting the patient home

With 24-Hour skilled nursing care and comprehensive rehab services we coordinate individualized care plans that fit the needs of each patient.
