Springfield Nursing & Rehabilitation Center would like to celebrate the importance of ethics, compassion and commitment in the health care profession by thanking our nurses, volunteers and entire staff for their hard work and dedication day in and day out.
Skilled nursing home care frequently begins with an on-site assessment at the hospital and continues through an initial conference and discharge planning. Our licensed professionals are available to answer questions and address any concerns you or your family may have.
As residents’ conditions progress and they move through different care levels, our staff works with their families and their physicians to transition them to the most appropriate senior care setting. Each resident’s short- or long-term care is customized for their individual needs and preferences, and delivered around the clock by trained nursing professionals.

Alisia Jackson, APRN
Nurse Practitioner
Alishia has been a nurse since 2006 and obtained her Advance Practice Registered Nursing License in 2018. Her medical expertise comes from experience in both the acute care setting and geriatrics as Alishia has worked in the long-term care setting for the last 3 years.
Not only does Alishia provide quality medical services to our residents here at Springfield Nursing, but she also provides Chronic Care Management (CCM) Services. The CCM component involves the creation of a resident-centered plan of care, medication monitoring, management of care transitions, electronic care coordination and the exchange of health information with other health care providers as necessary.
Alishia demonstrates and shares continued knowledge with our nursing team and shows true compassion for the residents under her care. She truly has a heart for her patients and loves what she does.
Penny Upton, LNHA
Administrator
Penny Upton brings more than 30 years of long-term care experience to her role as Nursing Home Administrator, with 12 of those years spent in administrator leadership. Her career is a true “from the ground up” journey—having served in nearly every department, including central supply, CNA, business office, and admissions. This hands-on background gives Penny a deep understanding of daily operations and the challenges faced by both residents and staff.
Long-term care is not just Penny’s profession—it’s part of who she is. Raised around the industry, with both her mother and grandmother working in long-term care, she developed an early passion for serving seniors and supporting those who care for them. That lifelong exposure shaped her commitment to advocacy, compassion, and quality care.
Penny earned her bachelor’s degree from the University of Phoenix, returning to school with a clear purpose: to make a meaningful difference. She is a strong advocate not only for residents, but also for staff, believing that empowered, supported caregivers are essential to exceptional care. Her leadership is rooted in respect, teamwork, and a genuine dedication to improving the lives of everyone within the long-term care community.


Cheryl Baker RN, DON, IP, WCC, RAC
Director of Nursing
Cheryl Baker, Director of Nursing, received her nursing degree in 1988, and brought with her a comprehensive background serving in Med-surgical, Intensive Care (ICU), Transitional Care (TCU), Emergency Room (ER), Coordination of Homehealth, Hand Surgery Clinic and Long-term Care.
She is certified as an Infection Preventionist, Wound Care Specialist, Resident Assessment Coordinator (RAC) and Deputy Register of Vital Statistic.
Cheryl’s love is in geriatrics, which is her main reason for choosing her nursing career. Her professional experience has made her a clinically strong nurse, who is deeply involved in health education, committed to clinical excellence, and lives by the philosophy that “the residents come first.”
Cheryl maintains an open-door policy for residents, families and co-workers and encourages involvement to ensure optimum resident care.
